Instructions
A. This application must be filled in typewriter OR ink, and submitted -- ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3.Approval of electrical installatlon from Board of Fire Underwriters.
4.Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings {prior to April 1957), Non-conforming uses, or buildings and 'pre-ex'st'rig"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

tion required to prepare a certificate.

a. This application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c, The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit

a. Foundations shall be 8", 10" 12" & 16" (as indicated on plans) poured concrete with

Exterior walls will be 2" x 4" wood studs 16" o.c. with Gypsum Board fastened to the
inside of studs. Outside to he covered with 1/2" CDX exterior pl~ood covered with
15~ asphalt saturated felt covered with finish materials as noted on plans & elevations.
All partitions~ exterior walls and ceilings will be covered with 1/2" thick Gypsum
Board, 5/8" Type 'x' firecode walls and ceilings i~ garage and over boiler or furnace.
Double all fr~u~ing under parallel partitions. Provide Pirestopping
around all openings as required by Code.
Provide at least double Joists and headers at water closet drains, and bathtubs, and
all vent and mechanical openings 2' or more in width~ unless otherwise shown in drwng.
Tail Joists over 4' lohg and header Joists shall be hung in approved metal stirrups
or hangers and spiked securely unless supported on a wall or girder.
Fasten securely all parts of carpentry work in their proper place. Brace, plumb and
level all members and secure with sufficient nails, spikes and bolts to insure rigidity.
h. Nail lapped Joists over any bearing, together with two 10d nails.
i. Provide solid surfaces at least 1-1/4 inches wide in both directions all corners for
securing sub-flooring, drywall, etc. Form surfaces with framing members or with 2 inch
wood blocking secure~ with at least two 8d nails at each end.
J. Provide 1" x 3" bridging between all floor Joists. Maximum space between rows of bridg-
ing and between bridging and supports shall be 8 feet.
k. Roof constl~ction shall he roof rafters covered with plywood sheathing and the roof
system consisting of 15 # base sheet covered with 240# asphalt self sealing shingles.
