Instructions
A. This application must be filled in typewriter OR ink, and submitted m~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p:Coperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion requ ired to prepare a certificate.

PERFORMANCE ASSURANCE -- Rugged nylon fibers are superior to all other carpet yarns for
abrasion resistance and retained good looks. Scheduled maintenance and periodic steam
cleaning keeps the carpet new.looking through the years of heavy commercial traffic.
SPILL AND STAIN RESISTANT .-- Dakota is treated with Scotchgard® carpet protector for that
extra insurance. Scotchgard~ actually repells most staining materials allowing time for clean-
up and routine maintenance.
STATIC PROTECTION -- Well'o's own static controlling agents are added to th~ carpet at the
factory. This means annoying cold and dry weather shocks are minimized, even after repeated
steam cleanings.
WEAR PROTECTION -- Wellco Business Carpet warrants Dakota for heavy commercial traffic
for a Period of ten years when properly installed and maintained. This is a limited warranty
which covers surface wear to the extent of loss of more than an average of 10% of pile fiber.
This extent of wear must be determined by an independent inspection agency. ~f the carpet
fails to perform as stated, the worn area will be replaced at our expense upon request of the
customer. This warranty does not apply to carpet insta!led on stairways. Also note: Chairpads
are necessary under office chairs with roller casters to preserve appearance, act as a deterrent
toward delaminatlon, and to avoid premature wear-out.

Please be advised that your Certificate of Occupancy
was subject to a one (I) year review by the Planning Board
in order to receive a'permanent C.O. This review was to
have been done April 14, 1987.
According to the Code of the Town of Southold Article
XXVIII Section 100-284 it is unlawful to occupy a building
without a Certificgte of Occupancy.
Please contact the Planning Board and the Building De-
partment to resolve this matter.

SPLASHBOARDS - Splashboards are No. 2 or better southern yellom pine 2"x8" $2S and center
matched, pressure treated to a net retention of .4 pounds per cubic foot with Chromated Copper
Arsenate Type III, Oxide type, in accordance with American Wood Preservers' A~sociatton
Specification C2. One rom ts furnished for building on a level grade.
FRAMING LUM~ER - Side nailers are 2"x4" S4S SPF 2100 FLSR spaced approximately 30" c~.:., with
al1 joints staggered at attachment to columns. Roof purlins are 2~x4'' S45 No. 2 SPF or better
on edge spaced approximately 20" o.c. All other framing lumber is standard grade or better.
ROOF TRESSES - Factory assembled ~ith 18 or 20 gauge galvanized steel truss plates ~ls required
amd graded kiln dried lumber as specified. Trusses are designed in accordance with current
s'candarcls and specifications for the stated loading.
BP~AC~NG - 0.035"x1%" high tensile steel diagonal, X-bracing in all corners. LateralS1 truss ties
and end bracing as needed.
~%~ROOFING AND SIDIRG PANELS - 0.019" min., Ggo zinc galvanized steel with an additioe~l baked-on
silicone finish. Color is I mil thick on exterior.
TRIM - Die-formed trim of 0.019" min. steel with silicone finish on corners, gable'~, ridges,
