Instructions
This application must be filled in typewriter OR ink, and submitted im,-,-.,~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-iS-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and sifnilar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1, Accurate survey of pZOperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

GENERAL CONDITIONS
1. The permiltee shall file in Ihe office of the appropriate Regional
~rmit Administrator, a notice of Intention to commence work at least 48
~urs in advance of the time of commencement and shall also notify him
omptly in writing oJ the complelion of the work.
2. The permitted work shall be subject Io inspection by an authorized
presentative of the Department Of Envltonmontal Cooservation who may

3. As a condition of the issuance of this permit, the applicant has ac-
~pted exp,essly, I~y the execution of the appllcalion, the full legal respon-
diity for ail damages, direct o, indirect, oJ whatever nature, and by whom-
er suffered, arising out of the project described herein and has agreed to
lemnity and save harmless the State from suits, actions, damages and

4. Any material dredged in the prosecution of the work herein permitted

8. That the State of New York shall in no case be liable for any damage
or injury to the structure or work herein authorized which may be caused by or
compensation shall accrue from any such damage.
