This application must be filled in typewriter OR ink, and submitted ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property [ines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building,
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-confo, rming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusua~ natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

tion 9aide for specific chimney require-
ments and always consult your local
budding authority regarding permits,

duced under the Factory Inspection

ers' Laboratories carry their Inspection
mark and have been tested in accor-
dance with American National Stan-
dard ANSI 131-1 fUL I03) (UL IO3-HT)
and CAN 4-S-60~ fULC S-604) or
UL C S-629M. Chimneys installed in

The AIA National Building Code, the
National Building Code of Canada,
C.S.A. and local build/nd codes,
CAUTION: An interior chimney is pref-
erable wherever possible. The contin-
uous "Deep Freeze" temperature to
which an extenor chimney is exposed
may result ~n operating problems such
as poor draft, excessive condensation
of combustion products and rapid
accumulation of creosote when con-
nected to a woodburning appliance
with a lower stack temperature.
One wood-burning
appliance
Any wood-burmng appliance should
have ~ts own chimney for safety and
for proper control. There are many
reasons for not connecting more than
one apphance to a chimney. If the
appliance is designed for open front or
open door use, so you can wew the
fire through a screen, it is a "fireplace"
and needs all the draft the chimney
can produce. Connecting an "air-tight"
or controlled draft h~gh efficiency wood
stove to a chimney which is also used
for a gas furnace may plug up the
chimney with creosote deposits. Th~s
may lead to carbon monoxide po~son-
~ng dthere is ~nadequate venhng when
the gas furnace ~s used.
The National Building Code of Can-
ada stipulates that engineered chim-
ney system installahons for use in
dwdlings constructed for three or
more families must be eac{osed
above the room In which the heating
apphance is located. This enclosure
must have a fire resistance rating
equal to or greater than that of the
floor or roof assembly through whmh
they pass
Clearance
Remember the clearances referred to
throughout this guide are the MINI-
MUM requirements, if addihonal space
is available - use ~t. The required
"clearances" are to be an un-obstructed
AIR SPACE ONLY - Do not fill these
areas wath any matenat especially in-
sulation. If re-insulating your attic area
in the future maintain this clearance.

System The entire system should be
inspected periodically and a regular
cleamng cycle should be determined,

a. This application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises (~r public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation. -
c. The work covered by this application may not be commenced before issuance of Building Permit·
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
