Based on my observations of the existing fiberglass insulation R3.5/in. for the as-built
second floor,basement and the existing plumbing for the two second floor bathrooms, it
is my opinion that they conform to the submitted permit plans and to New York State
code.

The purpose of this affidavit is to confirm certain facts regarding the home located at 1800
Westphalia Road, Mattituck, NY 11952,where I was raised as a child.
I lived in the home at 1800 Westphalia Road beginning in1963 and it was owned by my family
until it was purchased by the Halls in February, 2000.
This affidavit will clarify what was actually existing at the house while I was living there,
specifically as it relates to the condition of the second floor and the basement.

County).
The purpose of this affidavit is to confirm certain facts regarding the home located at 1800
Westphalia Road, Mattituck, NY 11952,where I was raised as a child. ,
I lived in the home at 1800 Westphalia Road beginning in1963 and it was owned by my family
until it was purchased by the Halls in February, 2000.
This affidavit will clarify what was actually existing at the house while I was living there,
specifically as it relates to the condition of the second floor and the basement.
The house was originally a 3-story barn that was converted into a house in 1962-1963. During
this conversion, the second floor was removed and the roof was lowered to create a 2-story
structure. The new second floor was improved with 3 bedrooms, 2 bathrooms and a room over
the kitchen below.
The basement was also finished at the same time. We built a room for the boiler,'a storage room
and a large open area.
