Instructions
A. This application must be filled in typewriter OR ink, and Submitted m ~aa,.aa~m to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal--{S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters. ~ -
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Re: Simicich - Bergen Avenue, Mattituck, New York
Dear Mr. Lessard:
Mr. and Mrs. John Simicich have applied for a certificate of
existing use for a barn and various sheds for tbeir farm property
located at Bergen Avenue in Mattituck. In this connection they
submitted to you an affidavit of William Lindsay, a long-term area
resident, to confirm that the barn and other out buildings had
pre-existed the zoning ordinance.
It is my understanding that since they have an above-ground pool
they will require a fire underwriters certificate in connection
with the electrical hook-up for ~be pump. Would.you please.
confirm that except for the requirement of the f~re underwriters
certificate, you would issue a certificate of existing use for the
barn and the other out buildings.
Mr. & Mrs. Simicich are in the process of obtaining fire
underwriters approval but this will takeapproximately' sixty (60)
days. In the meantime they wish to complete refinancing and the
bank wishes to confirm that a certificate of existing use will
issue once the fire underwriters certificate is supplied.

The following action was taken by the Southold Town Planning
Board, Monday, February 8, 1988.
RESOLVED that the Southold Town Planning Board take final
action on the minor subdivision of John and Catherine Simicich.
Board authorized the Chairman to endorse the final surveys for
this minor subdivision of 2 lots on 45.229 acres off Bergen Avenue
in Mattituck. SCTM# 1000-113-7-1.2, 19.161.
