A. This application must be filled in typewriter OR ink, and submitted -- ~,,-------- to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and instatla-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building,
5.Submit Planning Board approval of completed site plan requirements where applicable.

Dear Roy:
Re: ~86-SO149
I am enclosing a copy of the revised survey
as requested. When Dick Strouse was here last
week following our meeting, he revealed that the
western boundary of my property extends further than
we realized. With Mr. Horton, Ken Edwards and Dick
Strouse it was ascertained that the proposed dwell-
ing would best be moved somewhat to ~he west, per-
haps 20 feet further than we had agreed earlier.
This serves to bring the proposed dwelling further
back from the Pond and would allow the septic sys-
tem~to have a better location.
As you know I am extremely anxious to get
this whole matter settled and and will very much
appreciate your earliest attention to the matter.

Dear Curt:
Apropos of our conversation I am enclosing
a copy of the foundation plan for my proposed house
here at Fishers Island. You will note there is a
full basement called for under the original house
and crawl space under the kitchen addition. The
screen porch will have a slab foundation. Roy
Reynolds indicated to me yesterday that everything
had been approved in their department and I will
obviously send you confirmation to that effect
as soon as I receive it. Once you have received
that notification, is Lhere anything else that I
have to provide you with. I will call you on Mon-
day to double check on this.
I do appreciate your help and interes in
this prolonged endeavor.

Dear Mr. Horton:
I am enclosing an application for a
building permit filled out to the best of my
knowledge as discussed. This is to cover the
reconstruction of an 18th century house I am
bringing from Virginia. The house is supposed
to arrive in pieces the latter part of July and
we would like to start construction at that time
if possible. Obviously time is of the essence
and I would appreciate any help you might give me
to expedite approval of the building permit. I
have sent Mr. Reynolds of the Health Department
my application also.

APPLICATION FOR BUILDING PERMIT
INSTRUCTIONS
a. Tiffs application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector Will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
