This application must be fdied in typewriter OR ~nk, and submitted in duplicate to the Building Inspec-
tor w~th the following; for new buddings or new use
1. Final survey of property w~th accurate location of all buddings, property hnes, streets, and unusual
natural or topographm features
2. Fma~ approval of Health Dept of water supply and sewerage dlsposal-(S-9 form or equa$).
3 Approval of electrmal installation from Board of Fire Underwriters.
4. Commercta[ buildings, Industrial buddings, Multlple Residences and s~milar buildings and installa-
tions, a certificate of Code comphance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed s~te plan requirements where applicable.
For existing buddings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses'

As per our telephone conversation regarding
the premises at 360 Sunset Avenue, Mattltuck pertaining to
the ~ssuance of the Certificate of Occupancy, we do hereby
agree to the following terms:
1. We shall erect & ~nstall a temporary
walk and steps according to the bulldlng code and as per
your requirements.
2. In place of the aforementioned walks
and steps, we shall erect and complete a wood deck by
May 15,1987. Weather conditions permitting, completion of
this deck may be at an earlier date.
We appreciate your prompt attention to
th~s matter and look forward to receiving our Certificate
of Occupancy.
