APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the bumlding
inspector with the following: for new bumlding or new use:
1. Final survey of property wmth accurate location of all buzldings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certzfying that the solder used in system contamns
less than 2/10 of i% lead.
5. Commercial building, zndustrzal building, multiple re$zdences and smmmlar buzldings
and installations, a certzficate of Code Complmance from architect or engzneer
responsible for the buzldlng.
6. Submmt Planning Board Approval of completed smte plan requirements.
· For exzsting buildmngs (prmor to Aprml 9, 1957) non-conformmng uses, or buildzngs and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, buzldzng and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect szgned by the applmcant.
If a Certmficate of Occupancy is denzed, the Buildzng Inspector shall state the
reasons therefor mn wrzting to the applicant.
