APPLICATION FOR CERTIFICATE OF OCCUPANCY
Thms application must be filled in by typewriter OR ink and submitted to the building
inspector with the foliowzng: for new buzldmng or new use:
1. Final survey of property wmth accurate location of all bumldmngs, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of i% lead.
5. Commercial bumlding, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or englneer
responsible for the building.
6. Submit Plannmng Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, bumlding and
unusual natural or topographic features.
2. A properly completed application and a consent to znspect smgned by the applicant.
If a Certificate o£ Occupancy ms denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Greater than 300' from znventoried tidal wetlands.
Landward of a substantmal man-made structure
greater than 100' mn length constructed prior to September 20, 1977.
Landward of existzng 10' contour elevation above mean sea level on a
gradual, natural slope. There is to be no construction or disturbance
to vegetation or topography withzn 20' or seaward of exlstzng I0~
contour or topographical crest (as indzcated).
Landward of topographmcal crest of bluff, clzff or dune mn excess of
10~ in elevatzon above man sea level. There is to be no construction
or disturbance to vegetatzon or topography wzthzn 20' or seaward of

Therefore, no permit under Artmcle 25 (Tzdal Wetlands of the Envzronmental
Conservation Law) ~s required at thzs tzme since the current proposal ~s
beyond State mandated ]urisdictzon pursuant to thms act. However, any
