2, Fmal approval o~ Health Dept of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of F~re Underwriters.
4, Commercial buddings, Industrm[ buddings, MulUp[e Residences and similar buddings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the buddmg.
5, Submit Plannmg iBoard approval of completed s~te plan requirements where applicable.

corrected, and the Building Department should be finished with
their inspection on Tuesday, July 28th.

Gentlemen:
Per telephone Iconversatlon, I would like to modify my Trustees
Permit #2195 ~o include a deck in front of the cabin. The deck
would go up tq the existing bulkhead, and would be between
7 1/2' and 8' Isince it will just cover the top of the bulkhead
piles. The d~ck is approximately 2' above grade and will be 8'
wide and about 25' end to end. I'm not sure whether it is
going to be 251'6" or 25'10", depends on the grade. The deck
will not extend any further towards the wetlands than the
current bulkhead, so the change is basically for safety sake.
Instead of ha~lng stairs comlng down from the cabin, you will
step straight out.
I anticipate tlhat the entire project should be finished in
about another two weeks, at which time I'll not~fy you for the
inspection.

authorized b~ our General Permit for Non-Con~ercial Mooring Structures
{NYDGP-08} a copy of which is enclosed. Your work should be performed
according to the conditions of this General Permit.
Please display the enclosed Notice of Authorization sign at your work
site. A copy is enclosed for your records.
Notice is hereby given that the permittee should recognize that a
possibility exists that the structure permitted herein may be subject to wave
wash from passing vessels. The issuance of this permit does not relieve the
permlttee from taking all proper steps to insure the integrity of the
structure Pelrmitted herein and the tafety of boats moored thereto from damage
by wave wash. and the permittee shall not hold the United State's liable for any
such damage.
It should be noted that this permit does not relieve you of the
obligation to obtain any State~r local approvals for your work.
If, for any reason, a change in your plans or construction methods is
found necessary, please contact us immediately to discuss modification of your
permit. Any change must be approved before it is undertaken.

taken pursuant to this permit, and shall take all precautions for the prevention of injuries to persons
and property resulting from such operations By such acceptance, the applicant also agrees to indemni-
fy and save harmless tl~e Town, and ~ts officers, agents and employees from any and all claims arising
from operations under this permit and any and all acts or omissions of applicant, his agent and
employees.
The applicant and the owner and occupants of the premises upon which the operations authorized by

its officers and emplo!~'ees to enter upon the prermses where such operations are being conducted to
make such inspect~ons as the Town may deem necessary to insure that such operations are being con-

tation or the natural habitat thereof.
E. Increase the da~ger of food and storm-tide damage.
F. Adversely affect nawgat~on on tidal waters or the t~dal flow of the tidal waters of the town.
G. Change the course of any channel or the natural movement or flow of any waters.
H. Weaken or undermine the lateral support of other lands m the wctnity.
I. Otherwise adversely affect the health, safety and general welfare of the people of the town,
