Comments: (1) Current proposal does not conform to Department Standards. See enclosed Notice of Non-Conformance for more information. (2) Covenant language TBD if necessary pending BOR Determination. A date for the BOR hearing is TBD upon further review. (3) Submit a Water Design Report to demonstrate that the site will have adequate pressure under all anticipated flow conditions (i.e. the residence given the ~40’ elevation change & boat storage #2) at the furthest connection. Identify on plan the location of all water valves, thrust blocks, and hydrant & specify the diameter of the main. Submit hydrant
flow data with and without fire flow as part of this report. In addition, include the number of fixture units in the report, a riser diagram, & calculations for the needed fire flow using the ISO method. (4) Prior to SCDHS approval, sign-off from the Office of Pollution Control is required for the proposed abandonment of existing sanitary structure(s); contact Edward Roe at 631-
854-2501 for requirements. (5) The location of the proposed e-one pump station does not appear to be shown; revise to include. (6) Provide a design on the plan for the proposed retaining wall in accordance with standards. Revise sanitary profile accordingly. (7) Clarify if there are pump out facilities on the premises for the boats (specify if none); show location of any
holding tanks. (8) Show the location of the existing well for adjacent lot 13.7. (9) Submit the following paperwork: Certificate of Occupancy (COs) for all existing buildings to remain, a Certificate of Authorization from PWGC, & a SEQRA Determination from Town. (10) Submit a minimum of four (4) revised plans with original PE signatures/seals.

This application is not being processed for approval as submitted because it does not appear to conform with the standards/requirements of the Suffolk County Sanitary Code as indicated: 1. The proposed sanitary flow is greater than the allowable population density equivalent. A sewage treatment system is required. (Article 6, Section 607, Paragraphs A.1 and B.1, or A.2 and B.2 of the Suffolk County Sanitary Code). 2. The installation of a subsurface sewage disposal system(s) is not permitted when:
The system will be an insufficient distance from neighboring well(s); proposed well; and/or surface waters.
The subsoils on the site contain meadow mat, bog, silts, clays or other impervious materials which extend below the groundwater table or subsoils in the area are unsuitable for the proper functioning of the system.
The groundwater or drainage conditions are unsuitable for the proper functioning of the system.
A public sewer system is available.
3. The installation of a private water supply well is not permitted when:
A public water supply is available.
The groundwater supply in the area is inadequate for individual water supply wells.
The proposed well will be an insufficient distance from proposed or existing sewage disposal system(s).
The proposed sanitary flow is greater than the allowable population density equivalent.
4. The parcel(s) being applied on are apparently part of a realty subdivision or development, as defined in Article 6 of the Suffolk County Sanitary Code, which has not been approved by the Department for the method of sewage disposal and water supply. Prior to further processing of the application, it will be necessary that the owner(s) of the overall property file a realty subdivision or development map with this office. Application forms are enclosed.(1 & 2) 5. The proposal is not in conformance with Department construction standards.(3)
6. Installation of an on-site sewage treatment system is not permitted on land where community sewers are
required.
7. The proposal is not in conformance with the conditions of the approved realty subdivision or development map.
8. Other/Comments: (1) The proposal appears to be part of a realty subdivision without benefit of SCDHS approval. Approval of a subdivision application is required prior to issuance of a construction approval; see WWM-023 & WWM-203. (2) It appears a Board of Review (BOR) Application form was submitted, requesting a waiver from filing a subdivision due to the adjacent property owner unwilling to cooperate. The Department will need the owner (Jeffery L. Strong) to send a letter via certified mail to the adjacent property owner explaining why their cooperation in the
subdivision is required. The letter should include a section for them to sign with a response agreeing or refusing to participate. The Board will need a copy of this letter and their response prior to scheduling a hearing. (3) The density calculations provided do not include the flow from Boat Storage Buildings #1 & 2. A BOR variance is required if reduced flows are requested. In addition, the variance form provided does not appear to be from the Applicant/Agent. Submit a letter from the Property Owner requesting to add PWGC as a Design Professional. If this matter cannot be resolved, you may request a variance from these standards by applying to the Department’s Board of

(3) Based on a review of the submitted Water Design Report, clarification is required in reference to cross connection control and the proposed booster pump. No secondary RPZs appear to be shown or included in pressure calculations for marina workshops. Clarify how fire flow is being separated from domestic flow and how the single family
residence will be protected from any contamination from the other buildings. In addition, as per the Recommended Standards for Water Works, “Individual booster pumps shall not be allowed for any individual residential service from the public water supply main”. Verify if the water main or the single family residence water & show contours on plan for this area. Revise
Water Design Report accordingly for further review. (4) Prior to SCDHS approval, approval from the Office of Pollution Control is required for the proposed abandonment of existing sanitary structure(s); contact Edward Roe at 631-854-2501 for requirements. Revise Water Design Report to include needed fire flow with hydrant flow data.
The Department recommends the installation of an additional fire hydrant at the end of the line. (4) Prior to SCDHS approval, approval from the Office of Pollution Control is required for the proposed abandonment of existing sanitary structure(s); contact Edward Roe at 631-854-2501 for requirements. (5) Show the location of the existing well for adjacent lot 13.7. If the
cannot be found, then verify that the well is not over the property line and show a 150' minimum separation distance from the property line. (6) Submit Certificate of Occupancy (COs) for all existing buildings to remain. (7) Submit a SEQRA Determination from Town. (8) Submit a minimum of four (4) revised plans with original PE signatures/seals.
