A. This application must be filled in typewriter OR ink, and submitted m ~.~.- to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Dear Mr. & Mrs. Donadic:
This is to advise you that job under building permit
no. 11512Z issued to E.Z. Construction on December 11, 198!
for a one family dwelling for Steve & Marcia Donadic has
expired and a final inspection has not been done. We also
do not have your final board of health approval.
Also on May 2, ~984 building permit no. 13080Z was issued
to Steve Donadic to construct a patio and deck addition to
existing one family dwelling. This building permit expired
on November 2, ~985 and no final inspection has been made.
In order to complete these files, it is necessary that
Certificate of Occupancies be issued. Please fill out the
enclosed forms, return same to the above office with a check
for $50.00 payable to the Town of Southold. Please indicate
to whom the Certificate of Occupancies are to be mailed, and
arrange, with this office for an inspection date.
We are also enclosing building permit, application and
one set of plans along with your check #2539. Mrs. Donadic
submitted this application and was to send us two more sets
of plans that we never received.
Occupancy or us~ is unlawful without a Certificate of
Occupancy. Please h~lp us to clear up this matter so that
legal action does not ~have to be taken.

a. This application must be completely filled in by typewriter Or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
