Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res,
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building,
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic featu res,
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Dear Mr. Bellesheim:
During a review of our files, it was noted that the above
building permit has expired, and a Certificate of Occupancy has
not been issued.
According to the Code of the Town of Southold Article XIV,
100-141 and 100-144 a building permit is needed for construction
of an accessory swimming pool. It is unlawful to use said
swimming pool until a Certificate of Occupancy has been issued.
Please call this office and arrange for a final
inspection. A final inspection was made on 8/29/86 and CO was
denied because the gate needed to be repaired. Enclosed is your
Check ~133 in the amount of $5.00 dated 8/25/86. Please submit
a new check in the amount of $25.00.
This is your second notification. In order to avoid any
legal action you must correct this violation immediately.

Conservation Law) is required at this time since the current proposal is beyond
State mandated J~rlsdiction pursuant to this act. However, any additional work
or modifications to the project may require a permit. It is your responsibility
to notify this office, in writing, if such additional work of modifications are
contemplated.
