MOONEY, being duly sworn, deposes and says: I am one of the owners of 480 Rabbit Lane, East Marion, New York. I submit this Affidavit to the Building Department of the Town of Southold in order to secure a Certificate of Occupancy for that premises. I was the person who built the residence and who insulated the same. My recollection is that there was a full insulation inspection prior to the house being sheetrocked and the floor underneath the house being enclosed. I represent to the Town of Southold that the house is fully insulated and that the walls have insulation with a value of R-19 and the second floor ceiling and the floor under the first floor has insulation with a value of R-30. The house was built in 1985 and, hence, I do not have any records with respect to the purchase of the insulation. WHEREFORE it is respectfully requested that the Town issue the Certificate of Occupancy for the premises. STATE ENERGY C,ODE FINAL ADDITIONAL COMMENTS: ENGINEERS, CONSULT~TS, BUILDING INSPECTORS 516-549-2500 July 5, 1985 REPORT ON NEW HOUSE STRUCTURE at Rabbit Lane East Marion, New York On Wednesday, July $, 1985, an examination of the the home at the above address was made on behalf of Daniel Mooney of 89 Pershing Avenue, Locust Valley, New York. The purpose of the examination was in regard to some deviations from the original plans for the house and the structural integrity of the repairs made to correct problems. The inspection and evaluation ~f the new house structure repair effort was performed by the firm of Richard L. Heimer, P.E., P.C. with offices in New York City, Brooklyn, Huntington, Grpat Neck, Honsey, and White Plains, New York. Richard L. Heimer, P.E., P.C. is a consulting engineering firm professionally incorporated in New York State which is actively engaged in providing con- suiting engineering services to home and building owners, architects, and attorneys concerning new construction, reconstruction, rehabilitation, maintenance, corrective work and upgrading of homes and buildings. The President, Richard L. Heimer, is a Licensed Professional Engineer in New York (License #45882), as well as in New Jersey (License #GE 21629). He is a graduate engineer with a Bachelor of Science degree from the Massachusetts Institute of Technology (1956). He also is a graduate of Polytechnic Institute of New York where he received his Master of Science degree. He has 26 years experience as an engineer, estimator and inspector for construction projects ranging from single family homes to high-rise apartment houses to industrial and commercial buildings. Richard L. Heimer has lectured for the State Univer- sity of New York at Stony Brook, the C.W. Post Center of Long Island University, Hunter College, and the New York State Society of Professional Engineers. His experience and background have enabled him to qualify as an expert witness in hundreds of civil trials in state civil, state supreme and federal courts. Numerous articles about his work have appeared in local newspapers. He has also been interviewed on radio and television providing commentary and opinion. Richard L. Page 2 of 4 Mooney 7/3/85 66325 OBSERVATIONS The inspection was conducted in all accessible and observable areas and our specific findings were as follows: The east living room wall was originally called out in the drawings as a continuous 2x6 extending from the floor to the roof rafters (approximately 18 feet). However, during the construction this was changed to three 5x12 headers which was the typical of the construction utilized along the balance of the walls. This created somewhat of a problem. To provide the proper support, an additional 2x4 has been added along each stud tied in with gx4 braces. The new member is further anchored into the rafter above. Thus, providing a continuous member. We certify this as an acceptable repair alternative to the plans. The top plate on the 2x4's on the studs were finished with a single 2x4 on the bearing walls. The original plans called for two 2x4's. As an alternative a 2x4 has been added between each stud and an additional Zx4 was sandwiched in on each side of the original stud to provide support to the secondary cap. Tbus, making the overall wail stronger by tripling up on the studs. We certifiy this as an acceptable repair.
