APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter oR ink, and submitted m~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topogreph[c features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Su bm it Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pXt)perty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Upon application of EDWARD L. HERRMANN, lO00 Marlene Drive, Mattituck,
NY for a Variance to the Zoning O'~d'inan'c'e,'"'Article III, Section 100-31, for
permission to construct addition with reduction of sideyard setback to less
than the required 10 feet. Location of Property: 1000 Marlene Drive,
Mattituck, NY; 1000-144-2-40..
The board made the following findings and determination:
By this appeal, applicant seeks a variance to Article III, Section
100-31, Bulk Schedule, of the Zoning Code for approval of an insufficient
setback at nine feet from the southerly side property line in order to
construct a nine-foot by 24-foot addition for expansion of the present
living area. The premises in question is located at the west side of
~arlene Lane, Mattituck and is more particularly identified as Suffolk
County Tax Map District 1000, Section-~-44, Block' 2,~Lot 40, 'containing
an area'of. 1Q,875 sq. ft. with 75-foot.-ro~d frontage.'-The subject
prem{ses is improved with a one-family, one-story fra'me house and
~accessory frame garage located in the rearlard area and set back from
the northerly si. de property line 6.3 feet as shown on survey sketch
dated 9/7/~4, and 9/28J84. The perce, ntage of 'lot coverage for all
buildings, existing and proposed, is 19~.%, which is within that per-
mit%ed under the Bulk Schedule. The total Yari~nce requested by this
application is one foot, which, in'the board's opinion is not substantial
~.d is reasonable considering the'width of this lot. ~he total sideyards
of this dwelling structure w~ll be 34± feet, which is more than that
required under the zoning ordinance. The board agrees with applicant's
reasoning under the circumstances.
The'~board members have persona.lly visited the premises in question
and i~ familiar .with the buildings ~qd immediate area. It is noted
that opposition from the adjoining property owner on the south has been
made part of the record.
In considerfng this appeal, the board determines: (a) that
the relief as approved is no% substantial in relation to the zoning
requiremeDts (10%); (b) that by allowing the relief, no substantial

b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant, Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
