Instructions
This application muSt be filled in typewriter OR ink, and submitted ~ ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~t)perty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

GENERAL CONDITIONS
i. The permlttee shall file in the office of the appropriate Regional
Permit Administrator, a notice of intention to commence work at least 48
hours in advance of the time of commencemefll and shall also notify him
promptly in writing of the completion of the work.
2. The permitted work shall be subject to mspection by an authorized
representative of the Department of Environmental Conservation who may
order the work suspended if the public interest so requires.

tidal'wetlands ~eadow~ except a point where the driveway is to be 25' at
its closest p~int to tidal wetlands.

The following action was taken by the Southold Town
Planning Board, Tuesday, October 11, 1983.
RESOLVED that the Southold Town Planning Board approve
an amendment to Condition #3 of "Vincent Annabel Minor Sub-
division" to reduce the 10.0 foot setback to a 75 foot set-
back from the upland edge of tidal wetlands and shoreline
of Eugenes Creek for the construction of a residential
structure. .4

Dear Mr. Tashjian:
The following action was taken by the Board of Trustees at
their regular meeting held on ~Iay 29, 1986 regarding your
application.
Moved by Trustee Smith seconded by Trustee Larsen
WHEREAS, Edward Tashjian applied to the Southold Town Trustees
for a permit under the provisions of the Wetland Ordinance of the
Town of Southold, application dated March 5, 1986, and
WHEREAS, said application was referred to the Southold Town
Conservation Advisory Council for their findings and recommendations
and
WHEREAS, a public hearing was held by the Town Trustees with
respect to said application on May 29, 1986 at which time all
interested persons were given an opportunity to be heard, and
WHEREAS, the Board members have personally viewed and are
familiar with the premises in question and the surrounding area; and
WHEREAS, the Board considered all testimony and documentation
submitted concerning this application; and it is noted for the
record that there was no public opposition and
~HEREAS, the Board has determined that the project as proposed
will not affect the health, safety, and general welfare of the
people of the Town,
NOW, THEREFORE, BE IT
RESOLVED that Edward Tashjian be granted permission under the
provisions of the Wetland Ordinance of the Town of Southold,
New York for the construction of a 40' x 4' deck on premises
located on Stillwater Avenue, Cutchogue, New York with the
following provisions: 1. Add drywells to collect roof runoff.
2. Put in plantings to buffer the marsh. 3. Place heavy permeable
material (rocks or stones)under the deck. This permit will expire
on May 29, 1987 if work has not commenced by said date. The Trustees
are to be notified upon the completion of the work.
A $5.00 Wetland Inspection fee is now due and payable. Upon
remittance your permit will be forwarded by mail.

a. This application must be completely filled in by Wpewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
