Instructions
A. This application must be filled in typewriter OR ink, and submitted m~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Unde~vriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Dear Mr. Swanson:
At the request of Frank Laverty, Industrial Representative from'
Suburban Propane and for your company Laurel Greenhouses Inc., we
are pleased to submit our formal quotation for the propane storage
system.
SYSTEM DESIGN DATA
BTU per hour connected load - 5,100,000
Cubic Feet per hour connected load propane - 2,027
Gallons per hour connected load propane - 56
BTU per gallon propane - 91,500
BTU per cubic foot propane - 2,516
Specific gravity propane - 1.52
To provide a propane system based on the above design data, we
offer the following:
PROPANE STORAGE TANK
1-30,000 gallon water capacity used propane storage tank built in
accordance with the ASME code for unfired pressure vessels,
including National Board Number.
The tank will be fitted with a magnetic liquid level gauge,
indicating dial thermometer, 0 to 300 PSI pressure gauge, 85%
outage gauge, suitable excess flow valves and the necessary liquid
and vapor shutoff valves. The tank is approximately 66 feet long
by 108 inches in diameter. The tank will be equipped with 3-250
PSI relief valves complete with seven foot vent risers and
approved weather caps.

CONCRETE PIERS
Concrete piers will be provided to support the propane storage
tank(s). The concrete piers will conform to the shape of the
storage tank and will support the tank approximately four feet
above grade and approximately five feet below~grade.
1-Lot of concrete pads required to support above ground piping
wherever necessary will be furnished.
PIPING
Ail the necessary piping for both liquid and vapor lines and this
includes all Schedule 80 pipe and forged steel fittings will be
furnished. Piping will be provided from the transport unloading
riser to the tank and from the tank 100' to the building where we
will terminate with a valve on the outside.
TRANSPORT UNLOADING STATION
1-Transport unloading station fabricated in accordance with the
latest NFPA Pamphlet #58 specifications for vapor return for
liquid fill lines will be provided. The unloading station will
consist of a 2" indicating flow valve with back check, 2" strainer
and Acme adaptors with raincaps and chains. The vapor equalizer
line will consist of a 1-1/4" excess flow valve, 1-1/4" shutoff
valve and Acme hose adaptors with raincaps and chains.
FIRE PREVENTION
2 - 20 lb. dry chemical fire extinguishers will be provided
for the propane storage tank area, the transport unloading area
and other areas as required by NFPA Pamphlet #58.
1-Lot of "No Smoking" signs and "Special Instruction" signs will
be provided.
1-Lot of "Flammable Liquid" signs for mounting on the propane
storage tank will also be provided.
PAINTING
The entire installation will be painted and color coded. The
storage tank will be spot primed and finished painted SUBURBAN
CREAM. All liquid lines will be ORANGE and all high pressure
vapor lines will be WHITE. All piping will be adequately
indicated as to the direction of flow and to the contents of the
piping.

FENCE
A 6' high chain link fence with three strands of barbed wire will
be installed around the tank area and other ar-eas as required.
Also included will be two walk-through gates in each area.
ENGINEERING SERVICES, S~OP DRAWINGS AND DESCRIPTIVE DATA
Our quoted price includes all of the necessary shop and working
drawings such as plot plan, foundation drawing, piping drawings,
wiring diagrams and other pertinent data required for the
installation of the equipment. The drawings are usually available
in 2 to 4 weeks upon receipt of a formal purchase order.
OPERATING INSTRUCTIONS
A booklet containing operating, installation, maintenance and
start-up instructions will be furnished. All of the necessary
drawings required and a copy of NFPA Pamphlet #58 will be
included.
At the time of start-up of the plant a training session will be
conducted to train all operating personnel on the safe operation
of the system.
START UP AND ADJUSTMENT
Suburban Propane will furnish a field supervisor to supervise
installation of the entire system and to provide start up and
adjustment after the installation has been completed.

CRANE SERVICE
A crane will be provided to remove the propane storage tank from
the flatbed truck and install it on the concrete piers. This is
based on the provision that the crane and flatbed truck can enter
the job site and travel to the location of the proposed storage
tank installation under their own power.
TESTING
Ail piping will be pressure tested prior to putting the system
into service.

e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
