Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor w~th the following; for new buildings or new use:
1, Final survey of property w~th accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercml buddings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a cemflcate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planmng Board approval of completed s~te plan requirements where applicable.
For existing buddings (prior to Aprd 1957), Non-conforming uses, or buildings and "pre-existing"
land uses'
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

Pursuant to your request for a letter o£
clarification regarding the elevatio~mshown on the recent
map made for you of the Tuthill property at Mattituck, we
submit the following:
1. The elevations shown re£er to mean sea level, referred to
those shown on Su_zolk County topo map, but prepared from
actual field observations made by us. (While our original
map made for the Tuthills showed elevations referred to
high water mark, the new survey elevations have been con-
verted to the mean sea level basis).
2. With reference to the flood-zone map, we have plotted the
subject premisea.on,that map, and find that they lie
within the C-zone shown on said map~ with the northwest-
erly corner of the premises barely touching the southerly
end of the AS-zone, at which point we indicate an 8.0
elevation.
~. The flood-zone maps are prepared at a very small scale
(1000 ft. to the inch), and are used by us to the limits
allowed by the scaling, thus accounting for some possible
variations.
