Instructions
A. This application must be filled in typewriter OR ink, and sdbmitted in duplicate to the Building Inspec-
tor with the foilowJng; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal),
3, Approval of electrical installation from Board of Fire Underwriters.
4. Commercial tJuildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildibgs (prior to April 1957), Non-conforming uses, or bdildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features. ~
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

ZONING BOARD OF APPEALS
By this appeal, appellant seeks permission roi construct a
one-family dwelling with an insufficient frontyardl setback of
42 feet and rearyard at the east side of 35 feet. ' The premises
in question contains an area of approximately 21,2150 square feet.
The proposed dwelling will be approximately 64 fee!t long by 34
feet. Upon inspection, the. Board finds that neighborhood to be
nearly all vacant parcels.
The Board finds that the relief requested is .not substantial
in relation to the Code requirements; that the re'lief requested
is within the spirit of the zoning ordinance; tha~ the variance
if granted will not change the character of the n~ighborhood; that
no adverse effects will be produced on available governmental '~
facilities of any increased population; that the ihardship or
practical difficulty is unique; and that the interests of just-ice
will be served by granting the variance requested Isubject to
certain conditions, infra. ~
