Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable,
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:

Compensation Acts and from claims for damages because of bodily

or by any subcontractor or anyone directly or indirectly employed
by either of them. The Contractor shall carry not less than
$500,000 for injury and death to anyone or each person, and
$1,000,000 for all. Property damage not less than $200,000 on
account of darnages to others.
owner's Liability Insurance: The owner shall be responsible for

Fire Insurance: The Owner shall effect and maintain fire in-

with whether in or adjacent to the structure insured, materials
in place or to be used as part of the permanent construction.
Liens: The final payment shall not' be due until the Contractor
has delivered to the owner a complete release of all liens arising
out of this Contract; or receipts in fUl.l covering all labor and
,materials for which a lien could be filed, or a !;lond satisfactory
to the owner indemnifying him against any lien.
The Architect shall review and approve all requisitions for pay.

Cleaning Up: The Contractor shall keep the premises free ~rom
accumulation of waste materials and rubbish. Upon completion of
construction, the Contractor shall thoroughly clean the building
for all trades as follows:
1. Masonry: Clean with a stiff brush, removing all mortar stains,
dirt and other objectionable discoloration.
2. Glass: Remove putty stains from all glass, remove all paint
stains from glass, wash and polish same inside and out.
3. Hardware and other unpainted metal surfaces: ,Clean and
polish for all trades. This shall include removal of all
stains, dust, dirt, paint and the like.

EXCAVATING, GRADING, SITE WORK, SANITARY SYSTEM
1-01 EXCAVATION
Excavate to dimensions one foot greater in size than outside
dimensions of the masonry, slope to prevent cave-in, and to
the depth required, or to solid formation suitable for the
foundations"
The Contractor shall bring the grade up to the necessary level
to receive 6" top soil. Machine spread 6" of salvaged top
soil, balance of required top soil by Owner.
Excavation shall be kept free from standing water at all times.
Excess fill shall be removed from the site or distributed on
the site as directed by the Owner.

The Contractor will protect all existing facilities, which are

CONCRETE AND MASONRY
2-01 FOUNDATIONS
Foundations shall be constructed of hollow concrete block of
height shown on Drawings. All blocks shall be ~ell cured of
stone aggregate concrete, meetinq'A.s.T.M. spacifications.
Provide corner and solid blocks as shown or required. Mortar
for joints shall be one part Portland cement, two parts sand
to which 10' (by volume) lime shall be added. Blocks shall be
laid in full buttered joints and shall be set in straight hori-
zontal and vertical ceurses. Joints shall be staggered. The
top course shall be of solid concrete block units. Furnish and
install 1/2" diameter by IS" long ancher bolts for sill at !!Iini'.

The exterior of the foundation walls shall be dampproofed below
grade with two coats of asphalt emulsion over cement stucco
parging of the entire wall.

PAINTING
7-01 PREPARATlON
The Painting Contractor shall not commence work until surfaces
arB in proper condition. Ue shall protect adjacent work from
soiling or damage, as well as his own work. All surfaces to
be painted must be sanded smooth, clean and dry.
All knots or sap streaks shall be given one (1) coat of shellac
before painting. All necessary puttying of nail holes, cracks
and blemishes shall be done after priming coat has become dry
and hard before second coat is applied.
All mixing required ahall be done on the premises and no
materials shall be reduced or changed in any way except as
specified.
NOTE. All paints are to be of the color selected by the Owner.

This Contractor shall provide all labor and materials required
for the execution and completion of a satisfactory plumbing
system, with all necessary valves, fittings, vents, traps and
so for.th as called for and/or required by all applicable codes
and common good practice. All lines are to connect into exist-
ing system.

PIPING
.
Soil pipe below grade or below slabs are to be standard weight
cast iron, hub and spigot joints with packing and lead joints,
pipe coated with ooat tar pitch varnishl drainage, vent and
waste (above grade) copper, hot and cold water shall be hard'
copper tubing. Provide brass bodied valves wherever necessary
to properly control the various lines. Slope for drainage all
water pipes 1/4- per running foot back to basement, provide
drain cocks for hot and cold water at low po.int. Provide shut
off valvss on all water lines at fixtures.

All piping above basement shall be ooncealed in walls, exposed
piping at fixtures chrome plated brass. No.piping in exterior
walls.

ELECTRICAL
9-01 GENERAL
All electrical work, materials and methods of installation are
to be in strict accordance with local applicable codes and the
N.B.F.U. Code. Fire Underwriters Certificate shall be delivered
to the Architect prior to final acceptance.
9-02 ELECTRIC SERVICE
The existing overhead service is to remain.
9-03 DISTRIBUTION

Connect back to the nearest distribution panel.
9-04 MATERIALS
All wiring is to be copper, Romex, non-metallic, concealed in
construction. Switch and receptacle plates are to match exist-
ing. Convenience receptacles are duplex and switch silent type
. tumbler. Only one outlet of receptacles connected to switCh are
switch controlled. Color selection of switches, receptacles and
plates by Owner.
9-05 FIXTURES
Make an allowance of $250.00 (TWO HUNDRED FIFTY DOLLARS) for
electrical fixtures whiCh are to be selected by the Owner. In-
stallation by Contractor in his base bid.
Outdoor flood light lamp holders and porcelain bulb holders are
to be provided by the Contractor and are not included in this
Allowance.
9-06 TELEPHONE
Notify Owner when addition is ready for prewiring by the Tele-
phone Company to determine telephone locations in Master
Bedroom.
9-07 BATH FAN/HEAT LAMP/LIGHT
Connect exhaust duct to exterior. Use Nutone 9425F, two bulb
H.at-A-Vent, 70 cfm., provide three (3) switches for operation
of tan, light and heat lamp.

9-08 SMOKE DETECTING ALARM DEVIC~
Provide one single station smoke detecting alarm device adjaoent
to the bedroom, located on the ceiling. Device shall provide
visible indication that the alarm is energized. The device shall
be directly connected to the lighting circuit with no inter-
vening wall switch. The unit shall meet the requirements of the
New York State Building Code.

HEATING
10-01 SYSTEM
I. Connect ductwork to existing furnaoe.
2. The duct supply syste~ shall be through the floor with a
return in the Master Bedroom.
10-02 EQUIPMENT
Grilles, Registers and Diffusers - Lima.
10-03 DUCTWORK

1. The duct system shall be sized for air conditioning. Each
supply branch shall have a damper for balancing equipped with
quandrant and wing nut for locking in position. All duct-
work shall be straight and smooth and run between joists
where possible to maintain maximum head room. Ducts shall
be free from air leaks and shall be securely supported by
strap hangers fastened to sides of joists. All ducts shall
be insulated.

2. Fittings shall be streamlined for minimum friction and if
square elbows are uS.d, they shall be equipped with turning
vanes.
3. Connections to equipment will be made with fireproof fabric
connections to prevent transmission of noise or vibration
to duots.
