APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Construction on above described building and permit meets all applicable codes and regulations.
RESOLVED, that the applicant, William J. Boylhart, be and
hereby is granted a variance from the provisions' of the Flood
Damage Prevention Law of the Town of Southold, as applied for
in Appeal No. FL-11, to construct lowest floor of this proposed
one-family dwelling at 8' above mean sea level on premises located
at 1370 Latham Lane, Orient, New York, (County Tax Map District
1000, Section 15, Block 9, Lot 1.4), SUBJECT TO THE FOLLOWING
CONDITIONS, to wit:
[1] That the basement (below the 11' base flood level) be
floodproofed so that the structure below the base flood level
is watertight with walls substantially impermeable to the
passage of water;
[2] That the floodproofing requirements be certified by
a registered and licensed professional engineer/architect as
meeting the standards set forth in the preceding paragraph
hereof;.
[3] That such certification be filed with this board
(and with the building department) of the Town of Sou~hold;
[4] That prior to commencement of construction, the
applicant apply for and obtain a Development Permit from the
Town building inspector, pursuant to the provisions of the
Flood Damag~ Prevention Law of the Town of Southold;
[5] That the basement floor not be constructed less
than eight feet above mean sea level;
[6] That the basement/cellar area is not to be used for
habitable purposes, only storage being permitted;
AND IT IS FURTHER RESOLVED, that pursuant to the provi-
sions of Section 46-16F of the Code, the applicant is hereby
given notice that the structure for which this variance is
granted will be permitted to be b~ilt with the lowest floor
elevation below the base flood elevation as indicated above
and that t~e cost of flood insurance will be commensurate
with the increased risk resulting from the reduced lowest
floor elevation;
AND IT IS FURTHER RESOLVED, that the Secretary to this
board transmit copies of this determination to the applicants
and to the Town building inspector.

This is to certify that the foundation, as designed for the subject
residence, complies with the Southold Tewn ordinance, paragraph 16-18B, as
follows:
46-18B (1) is satisfied based on the classification of building structure
FP-~ in conjunction with space classification¥~2 under the definition of
"Essentially Dry" as defined in the publics£ionEP 1165-2-3~4, "Flood
ProOfing Re~c~lations" published by the U. S. Army Corp of Engineers,
dated Junef 1972.
The structure as designed also satisfied with criteria stated in paragraph
46-18B (2) for resistance to the effects of buoyan.cy, hydrostatic, and
hydrodynamic pressure for A zones. In particular, the design provides for a
reinforced pier foundation with the top of the foundation at 11 feet MSL,
with the foundation water proofed to B type as defined in section 502 of
reference ~1.
Based on the Variance gr~ltedby the Sou%hold Town BoardofAppeals, ref-
erence ~2, a basement was allc~ed below the BFP so lonq as the floor was
above 8 feet ~L.
The location of mechanical equipment was located below the RFD as provided
for in section 1301.2 of reference ~1.

Painting

3 - Sand, finish and minwax oak floors, first floor only - sand
and finish stairs.
4 - Figure cost of entire painting, staining and finishing of
interior.
