APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buddings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topograph m featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

We a~e a medium size congragation but a yery determined one, and with the help of

given to us. The Pastor and members of Jefferson's Temple have been working for
many years towards this special occasion.

Dear Rev. Jefferson:
The following items must be taken care of in regard
to the site plan for your new church.
The church must be set back 50 feet from the road.
Parking stalls must show ten feet width. Curb cuts
must be obtained from the Suf£olk County Department of
Transportation. There must be ten feet of buffering on
the sideyards. An update from the Suffolk County Dept.
of Health is necessary.
The Planning Board would also like to determine
where the property line is in reference to the existing
church.

Gordon Ahlers and have him take care of the matter for

construct asphalt pavement for 2 accesses and place topsoil ancl seed
as per revised site plan and typical section on the n/s/o .~iddle Road,

NOW, THEREX~ORE, permission is hereby granted to said
REV. J0tAX7 J. JEFFfIRSON to do said work upon the following conditions
CONDITIONS AND RESTI%IC~I'IONS
L When concrete is removed, cut concrete with concrete saw two feet outside the
limits of the proposed excavation and for the full panel width. When the cut is
made in pavement wttldn 10 feet of the panel Joint, the pavement shall be removed
for full panel width from the cut on the far side of the excavation to the panel joint.
2. Sack/iH and thoroughly compact fill using POWER TAMPER, pla~lng soils so that
various types of soil are in saxne relative position as before excavating. Grade to
original surface.
3. Repl~ pavement, sidewalk or surface same as existing; if concrete, replace using
1:1 ~ .3 ~/z mix stone mix concrete; ff bituminous material, replace using samae type
$, If coneret~ curb Is removed, replace with similar curb 1:I~ :BI/~ mix,
5. Edge of Jacking Pits or any other excavation Shah be not less than 5 feet from Edge
of Pavement. No undercutting allowed in Pit.

This permit shall not be assigned or transferred without the written consent of the Commissioner of Public Works.
The work authomzed by thru permit shall be performed under the supervision and to the sahsfaction of the Com-
missioner of Public Works or his representative.
Particular attention is called to the necessity of thoroughly compactmg the bacg fill, which will be required
by the Commissioner of Public Works.
The Commissioner of Pubhc Works shall be given one week's notice by said applicant of the date when it in-
tends to begin the work authorized by this permit, and prompt notice of ~ts completion.
The said applicant hereby agrees to hold the State, County and Town harmless on account of damages of any
kind which may arose during the progress of the work authomzed by this permit or by reason thereof.
Applicant certifies all persons concerned w{th actual work under thru permit are duly covered by Workmen's Corn-
pensation Insurance and the State, County and Town shall be held harmless on account thereof.
The Commissioner of Public Works reserves the right to at any time revoke or annul this permit should the
said applicant fail to comply with the terms and conditions upon which it is granted.
The applicant agrees to pay all necessary expenses incident to supervision and inspection by reason of the
granting of such permit as certified by the Commissioner of Public Works, such payment to be made within ten days
from the rendering of the certified account.
Work under this permit to be commenced within thirty days from the ~ate of permit and continued in an
expeditious manner.
The applicant shalI su,bmit a detailed plan of structure to be built, with a description ~ proposed method of
construction.
It is understood that sbeuld future changes in the alignment Qr grade of the highway require changes in the
work covered by this applicahon and permit, the applicant shall on reasonable notice from the Commissioner o! Public
Works make such necessary changes at b~s own expense within the time so specified in notice.
Traffic shall be maintained by the applicant on this section of the highway while {:he work is in progress and
until its final eempletlon.
A CertifiedBond CheekCash in the sum of $.~.-.*..0.0. ~.*.0..0. payable to the County Treasurer of Suffolk County is
on file
deposited as security that the highway will be restored to its origmal condition where disturbed at the expense of the
applicant, as soon as the work has been completed, and the said Comrmssioner of Public Works is hereby authorized
to expend all or as much of such deposit as may be necessary for that purpose, should the said applicant neglect or refuse
to perform the work.
In consideration of granting this permit the undersigned accepts it subject to conditions described.
