Instructions
A. This application must be filted in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of itealth Dept. of water supply and sewerage disposal-rS-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topograpi~ic features. ~
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

On February 16, we made a visual inspection of the heating system
at ~ir. Seaman's house in Southold. We also l~rformed combustion
efficiency tests before and after burner adjustment. The results
of these tests are as follows;

We repaired the front wall of the combustion chamber, which had fallen
forward and was causing an oil impingement.
In figuring the heat loss of the building, we used a design temperature
difference of 80°, which is equ~l to an outside temperature of -10°.
This is in excess of the normal Long Island design requirements. At
this design temperature, the house requires 35,150 BTUH. i The installed
boile~ will deliver 87,000 BTUH Net output, and the installed radiation
is equal to#5,030 BTUH. A copy of the heat loss calcolationhas been
included for your records.
It is my opinion that the boiler and radiation which are installed in
th~s house are mox~ than adequate to provide the standard ?0° inside
temperature.
