· APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate lOcation of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead. ~
5. Commercial building, industrial building, multiple residences an~ similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.'
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. -Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and'~ consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

As per my telephone conversation with your department
these are the following changes which I am allowing
on the above mentioned residence.
1. Run two two by tens (2 x 10) from the wall next to
the angled bay in the dinette left across the plan to
the outside wall by the outside bath. Shift the
window in the bath up on the plan to allow for bearing
of this beam.
2. The fresh air supply to the fireplace shall be
placed in front of the glass doors.
3. Substitute all steel beams in the basement for three
two by tens (2 x 10) built up. Add an additional lally
column and footing in the beam line under the bedroom
wing and under the living room wing. Add two additional
lally columns under the besm in the area where the
foundation breaks (beam under wall between hallway and
dinette).

Inspector, with 3 sets of plans, accurate plot plan to scale. Fee according to schedule·
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Pe~it.
d. Upon approval of this application, the Build~g Inspector will issue a Building Pe~it to the applicant. Such pe~it
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in pa~ for any pu~ose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
