BOX 728 TOWNHALL SOUTHOLD, N.Y. 11971 August 26, 1986 Dr. David Eilbert North Fork Optical A & P Shopping Center Main Road Mattituck, NY 11952 Dear Dr. Eilbert: Thank you for the cooperation during the fire inspection of the North Fork Optical Shop the latter part of July 1986. The occupancy load of your shop is less than 49. The enclosed certificate is to be posted in a conspicuous location in the public area of the store. During the -inspection the following were noted and should be corrected as soon as possible. 1. There shall be an illuminated Exit sign over both doors. The back door shall be equipped with emergency egress hardware that will not impede exit at any time. Solid bars, bolts, locks, or any hardware that stops egress through the door at any time is illegal and not allowed. 2. All exit lights must be illuminated any time that the store is open to the public. 3. There shall be a minimum of one (1) set of emergency exit lighting positioned so as to illuminate the exit, paths in case of a power interruption. 4. Fire extinguishers shall be provided in proper number and type. These shall be mounted in an accessible position and clearly marked. These extinguishers shall be inspected monthly by a designated employee and yearly by an approved extinguisher supply company. 5. Storage of materials and combustable liquids must be maintained in proper amounts and stored in a neat and orderly fashion. 6. Store #6 A & P Shopping Center 1. Existing exit signs are not maintained in operable condition. 2. Emergency exit^rear of store has been removed, west side (replace). 3. Directional sign in passageway of westerly rear exit is required. 4. Passageway to westerly rear exit was obstructed with products recently delivered (passageway and corridors used for emergency egress must remain clear and unobstructed at all times). 5. Rear exit doors were key locked (doors shall remain unlocked during the hours store is open to the public or locked with an approved type locking device). Store #12 - Variety Store 1. State Construction Code. I have reviewed the outstanding building permits and made an inspection of stores under these permits. I have prepared a list of what is required to bring these stores up to code. I have also made an inspection of some of the stores that have been operating for some time and found that fire safety requirements are not being maintained, i.e.: exit marking signs are not lighted, mainly due to burned out bulbs; the sporting goods store has been partitioned and the access to the rear emergency exit has not been identified; the Spirit shop has the emergency exit at rear of store barricaded during their business hours. A barricade such as this is not an approved locking device and must be removed. - - I shall be going over the enclosed list with you to discuss any questions you may have. A date for compliance to the items on this list will have to be set. Within this time frame all existing occupancies shall have to be brought up to code rela- tive to fire safety.
