Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the followlng; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-{S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code comp[iance from the Architect or Engineer responsible for the buitding.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"_

1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or

A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.

4. Sworn statement from plumber certifying that the solder used in system contaihs
less than 2/10 of i% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer-
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements. ·
B. For existing buildings (prior to April 9, 1957) non-conforming uses or buildings and
"pre-existing" land uses: '
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Mr. Hindermann:
At the request of Roy Rosenbaum, Architect, for the subject
project, I made a personal inspection of the project on April
29th, 1983 to determine the cause for the collapse of the founda-
tion walls and to make recommendations for the re-building of
said walls.
The failure of the walls was not due to any structural de-
ficiencies but rather to the failure oflthe Jo~ Superintendent to
adequently brace the walls before back-filling ~ook place. My
recommendations for the re-building of the foundation walls and
footings was to first remove all concrete which had collapsed or
moved out of its original locations by more than 3/4". After the
concrete has been removed, new footings shall be poured where re-
quired with the Key-way as shown on the original drawings plus the
installation of vertical steel reinforcing bar dowels spaced 2'0"
o.c. Where near-vertical or angular surfaces remain on existing
foundation walls which were left standing, horizontal holes will
be drilled into existing concrete and No. 5 steel dowels inserted
and properly anchored 2'0"o.c. from the top of the walls to the
bottom. These dowels should protrude approximately 18" from the
existing concrete so when the new foundation walls are poured, the
dowels will provide a physical bond between the new wall and the
existing.

Also, before any new concrete foundation walls are poured a "Bondcrete"
or equal product should be ~_p.~ied to the existing concrete to provide
for any additional physical bonding.
In my opinion, if the procedures as outlined in the previous
paragraph are followed, the foundation walls should be structurally
adequate and in general compliance with the concept of the original
structural design prepared by Roy Rosenbaum, Architect.

Enclosed herewith are the energy calculations for the above
mentioned project. These computations are in accordance with
the New York State Energy Code.
Please note that we have provided additional insulation to
that previously indicated on plan in that there is Bart
Insulation which is being provided on the walls; the cores
of the concrete ~locks .have being filled with insulating
Vermiculite. We will also be adding Batt Insulation on the
portion of Masonry wall between ~he hung ceiling and the
underside of the roof deck.
Please note that in accordance with table 4-2, the minimum
"R" value for basement walls below grade is 5.0 and "R" value
of 12" of concrete is over 5, therefore no insulation on

Dear Mr. Lessard:
Attached please find copy of correspondence from Mr.
Burt Seelig, with regard to the above captioned site plan.
Based on this correspondence and telephone conversations
with him and Mr. Hindermann, each of our Board members
were contacted by telephone for their input regarding the
approval of a temporary CO for a period of 60 days, during
which time necessary improvements will be completed. Mr.
Seelig has also advised our office that condition number
three will be resolved with the Planning Board So that a
permanent CO can be issued.
It was the concensus of the Board to allow the issuance
of a temporary certificate of occupancy for a period of 60
days for Cutchogue Shopping Center, Suffolk County National
Bank, based on the correspondence received and the telephone
conversations with Mr. Seelig and the Building Department.

Dear Mr. Raynor:
We are in receipt of your letter dated September 15th,
1983.
We have no difficulties complying with the conditions
in your correspondence except for Item No. 3. It is our
contention that the removal of the concrete island north of
the central ingress and egress will create a traffic hazard.
The State of New York, Department of Transportation in their
letter to you dated August llth, 1983 agrees with our opinion.
For your convenience, we enclose herewith a copy of that cor-
respondence.
We therefore respectfully request that condition No. 3
previously referrred to be eliminated.

PROTECTIVE LIABILITY INSURANCE COVERAGE
lSermittee must have protectiv~ liability insurance coverage in accordance with Department requirements. (See Infor-
mation on Protective Liabilityilnsurance Required for Highway Work Permits and/or Special HauLing Permits, Form
PERM- 27).
Expiration of, or lack of, liability insurance automatically terminates the permit.
Insurance coverage may be provided by furnishing the Department with one of the following:
a. A Certificate of Protect ve Liability Insurance for permits on State Highways (Form PERM 17, NYSDoT).
b. A $2.50 remittance (check drawn on a New York State Bank or Certified) for coverage under the Departmental Blanket
Policy.
c. 'An Undertaking may be furnished by Public Service Corporations and Government Units only and must include the
wording required by the D~partment.
COMPENSATION INSURANCEI AND DISABILITY COVERAGE
The applicant is required to have compensation insurance and disability coverage as noted in the provisions ofthe
Workmen's Compensation Lawand Acts amendatorythereof forthe entire period of the permit, or the permitis invalid.
NOTIFICATIONS
Notify Commissioner, through,Regional Office, one week prior to commencing work, except emergency work by public
service utilities which should be reported the next work day.
Work must start within 30 days from date of permit.
No~ify area gas distributors 72 hours prior to any blasting.
Notify Utility Companies with facilities in work area (permission must be obtained before doing work affecting utilities'
facilities) before starting work.
Notify land owners of abutting lands, before disturbing trees.
Notify Department of Transportation at conclusion of work and return original copy of permit to Regional Office.
Annual Maintenance Permit Notifications:
Notify by telephone the District or Resident Office, one week in advance, each time regular maintenance work
is to be performed. In emergencies, notification by telephone should be made the next work day.
SITE CARE AND RESTORATION
An Undertaking, a bond or ceitified check in an amount designated by the DepartmentofTransportation may berequired
by theRegionaiOffice, bef°re ;a permit is issued, to guarantee restoration of the site to its original condition. If the
Department is obliged to rests>re the site of its original condition, the costs to the Department will be deducted from
the amount of the permittee% guarantee deposit at the conclusion of the work.
The permittee is responsible for traffic protection and maintenance, including adequate use of signs and barriers during
work and evening hours. Any6ne working within the R.O.W. will wear an orange vest and hard hat.
No unnecessary obstruction is to be left on the pavement or the right of way or in such a position as to block warning
signs during or between work hours.
No work shall be done to obstruct drainage or divert creeks, water courses or sluices onto the right of way.
All falsework must be removed and all excavations must be filled in and restored to the satisfaction of the Regional
Traffic Engineer.
COSTS INCURRED BY ISSUANCE OF THIS PERMIT
All costs beyond the limits o;f the protective )lability insuronce~ surety deposits, etc., are the responsibility of the
permittee.
The State shall be held free of any costs incurred by the issuance of this permit, direct or indirect.
SUBMITTING WORK PLANS,
The applicant will submit wdrk plans and/or a map as required by the Department. This shall include such details as
measurements of driveways with relation to nearest corneh positions of guys supporting poles and a schedule of the
number of poles and feet of excavation necessary for completion of the work on the State right of way. A description
o4 the proposed method of construction will be included.
Plan work with future adjustments in mind, as any relocation, replacement or removal of the installation authorized by
this permit and made necessary by future highway maJntenance~ reconstruction or new construction~ will be the respon-
sibility of the permittee.
The permittee must coordinate his work with an}, state construction being conducted.
TRAFFIC MAINTENANCE :
Traffic shall be maintained bv the permittee on the hJghwav, in a safe manner, during working and non-working hours until con-
struction is completed. Suitable safeguards, to reduce conditions dangerous to life, limb and property to a minimum, must be pro-
vided by the permittee (includi'ng flagmen when requested by the Department).
COST OF INSPECTION AND SUPERVISION
Jr, in the accounts kept by the Department, costs of supervision ore found to b:e exceedingly high, the Department re-
serves the right to bill the ~ermittee for actual expenses incurred by the supervision and inspection of the permittee's
project.
