A, This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disp0sol--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, /V~ultiple Residences and similar buildings and
instalJatlons, a certificate of Code compJJence from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where ~applicoble.
B. For existing buildings (prior to April 19S7), Non-conforming uses, or buildings and "pre-existing"
~and uses:
1. Accurate survey.of property showing all property lines, streets, building~s and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3, Date of any housing code or safety inspection of buildings or premises, or other pertinent
formation required to prepare a certificate.

Proposed new dwelling
Dear Miss. Teich:
On June 17, 198B, we received an application from Mrs.
Kelsch to construct a one-family dwelling on premises located
on Wendy Drive, Laurel, (County Tax # 1000-127-08-019).
The application, plans~ and required permits from other
agencies involved have been reviewed and found to be in order.
Due to the back log of building permit appliations, we
were unable to issue a permit when applied for, however, a
verbal approval was given to start construction. Their appli-
cation has been slated for issuance, and when their's is
reached on our list the permit will be issued.
