APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Gentlemen:
During a review of our files it was noted that the above
building permit has expired, and a Certificate of Occupancy has
never been issued.
A final inspection was completed April 18, 1985, but the
CO was never applied for. Enclosed is the application. Please
submit it to this office along with the Underwriters Certificate
and a check in the amount of $25.00.
According to the Code of the Town of Southold, Article
XXVIII 100-281 and 100-284 a building permit is needed for any
construction of additions, accessory structures or new
dwellings, and it is unlawful to occupy or use said structure
until a Certificate of Occupancy has been issued.
If you have any questions regarding the above, please do
not hesitate to contact this office.

Dear Mr. Horton,
Reference Mr. Lessard's letter of February 10, 1993 pertaining
to our expired building permit and underwriters certificate. This
is our second attempt to close the permit but do to lack of
certification on our fire wall it has not be possible to do so.
Mr. Lyons visited our building on Monday March 15th and stated
that if the Town Building Department approves the fire wall he
would issue a certificate immediately. Your office would have to
submit a letter, signed by someone of authority, approving the wall
before he can proceed any further.
I believe that you were satisfied with the wall after your
last personal inspection and that we have complied with your
requests. We therefore request that a letter be sent to the Board
of Underwriters approving the wall.
Thank you for you past and present cooperation and we are
awaiting your reply.

The public hearing on this matter was held on .July 29, 1982.
Upon application of MATTITUCK AIRBASE INC., Airway Drive, Mattituck,
NY for a Special Exception to the Zoning Ordinance, Article VIII, Section
100-80B(1) for permission to alter and construct addition in a "C-Light"
District located at 410 Airway Drive, Mattituck, NY; bounded north by New
Suffolk Avenue and wickham; west by Bagshaw, Peconic Corp. and Wickham;
south by Park Avenue; east by Wickham, Parkin, Tandy, Wickham and Hotman;
County Tax Map Parcel NO. 1000-123-1-2.
'The-board made the following findings and determination:
By this appeal, appeilant seeks permission to construct a 30' x 125'
addition attached to the existing shop building, leaving a rearyard set-
back of approximately 59' from the rear property line (presently abutting
~Bagshaw). The premises in question is trapezoidal in shape and has a
-frontage along Airway Drive of 260 feet. Existing on the premises are
the principal building and an accessory building located in the northerly
sideyard area. The principal building is extensive, with an office,
stock rooms, waiting room
For the record it is noted that appellant has previously applied
to this board under Appeal No. 2893 for permission to construct an
addition with an insufficient fron~tyard~setback for office space. That
variance was given a conditional approval at a Special Meeting held
November 7, 1981.
In considering this appeal, the board determines that the variance
request is not substantial; that the circumstances herein are unique and
practical difficulties have been shown; that by allowing the variance no
substantial detriment to adjoining properties would be created; that the
difficulty cannot be obviated by a method, feasible to appellant, other
than a variance; that no adverse effects will be produced on available
governmental facilities of any increased population; that the relief
requested will be in harmony with and promote the general purposes of
zoning; and that the interests of justice will be served by allowing the
variance as indicated below.
On motion by Mr. Douglass, seconded by Mr. Goehringer, it was
