A, This application must be filled [n typewriter OR ink, and submitted,~;~l~-~teto the Building Inspec-
tor with the following; for new buildings or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage dispose[-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peopert¥ showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion requ!red to prepare a certificate.

New York State Department of Environmental Conservation has found the
__parcel x project to be:
Greater than 300' from inventoried tidalwetlands.
Landward of a substantial man-made structure (funotional.~ut~$~)
greater than I00' in lemgth constructed prior to September 20, 19~7.
Landward of 10' contour elevation above mean sea level on a gradual, nat-
ufa1 slope.
...... Landward of topographical crest of bluff, cliff or dune in excess of 10
feet in elevation above mean sea level.
Therefore, mo permit under Article 25 (Tidal Wetlam~Is of the Environmental
Conservation Law) is required at this time since the c~urrent proposal is beyond
State mandated J.(~risdiction pursuant to this act. However, any additional work
or modifications to the project may require a permit. It is your responsibility
to notify this office, in writing, if such additional work of modifications are
contemplated.
