APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Depa~ment with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance fi.om architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) hon-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Boufis,
On October 21, 2002, you inspected the premises at 50 Wabasso Street, Southold in
response to an application for a Pre-Existing C nfO submitted by Mrs. Beverly Burkhardt, the
executor of the Estate of her late husband William H. Burkhardt.
Your report of that inspection(see enclosed copy) required that the accessory garage
comply with Southold ZBA ruling/t3190 dated December 28, 1983 as a prerequisite to the
issuance of the Pre C nfO. A subsequent inspection by you (see enclosed) performed on January
27, 2003 found that all requirements for the Pre C of O had been met with the exception of a
clarification of whether or not the existing electrical service could remain in the garage.
The ZBA declined to say whether its December 28, 1983 ruling required the removal of
electricity from the garage. Mrs. Burkhardt was faced with choice of either applying for a new
variance from the ZBA or the removal of the electrical service. She chose the latter option.
Enclosed you will find a letter from LIPA stating that the electrical service was removed from the
garage on May 14, 2003.
With the removal of the electric service from the garage, all requirements for the issuance
of the Pre-C of O have been met. If there is anything else you need before you issue the Pre C of
O, please feel free to contact me at 477-2700. Otherwise, I look forward to a prompt issuance of
the Pre C of O.
