-APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposol--(S-9 form or equal).
3. Approval of electrical installoti0n from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, o certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey-of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Dote of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

INSTRUCTIONS
a. This application must be completely filled in by typewriter o~ in ink and submitted in triplicate to the Building
Inspector, with 3 sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and giving a detailed description of layout ofproperty must be drawn,on the diagram which is part of this application.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions or alterations, or for removal or demolition, as herein described.
The applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to
admit authorized inspectors on premises and in buildings for necessary inspections.

Dear Ms. Gasparik:
Thank you for the cooperation during the fire inspection of
the Super Value Variety Store the latter part of July 1986.
The occupancy load of your store is 49. The enclosed
certificate is to be posted in a conspicuous location in the
public area of the store.
During the inspection the following were noted and should be
corrected as soon as possible.
1. There shall be an illuminated Exit sign over the
front door. Both front doors shall be unlocked and
usable any time the store is open.
2. Ail exit lights must be illuminated any time that
the store is open to the public.
3. There shall be an illuminated "To Exit" sign over
the door into the back room. There may be an "Employee
Only" sign on the door.
4. There shall be an illuminated Exit sign over the
back door. This door shall be equipped with emergency
egress hardware that will not impede exit at any time.
Solid bars, bolts, locks, or any hardware that stops
egress through the door at any time is illegal and not
allowed. '
5. There shall be a minimum of one (1) set of emergency
exit lighting positioned so as to illuminate the exit
paths in case of a power interruption.
6. .The storage in the back store room shall be arranged
in such a way so as not to restrict access to the back
exit, the electrical service panels, and the heating and
cooling systems.

7. Fire extinguishers shall be provided in proper
number and type. These shall be mounted in an
accessible position and clearly marked. These
extinguishers shall be inspected monthly by a designated
employee and yearly by an approved extinguisher supply
company.
8. The use of unvented portable heaters using solid,
liquid, or gaseous fuel is prohibited in any retail
store or area of public assembly.
