Instructions
A. This application must be filled in typewriter OR ink, and submitted as ammorrr to the Building Inspe,
for with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusui
natural or topographic features.
2. Final approval of Health Dept, of water supply and sewerage disposal—(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and install-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the buildin,
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent inform
tion required to prepare a certificate.

1 . Application for updated CO.
2 . Check in the sum of $50 . 00.
The original building permit that was issued to build this
house with an attached garage was 9277Z, When Howard Terry
issued the CO on January 26, 1978 #Z8917 he only issued it
for a "PRIVATE ONE FAMILY DWELLING" and did not include WITH
ATTACHED GARAGE. I am advised that the building plans in
the file showed that the attached garage was always a part
of the house.
I would appreciate it if this can be take ca e of as
quickly as possible as we are ready to hedule a closing
and are unable to do so without the up ted CO.
