Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposol--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, o certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey aT property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.

4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and

Dear Victor:
I am enclosing herewith a "Set Off Map" which has been approved by the
Health Department and the Southold Town Planoing Board. Also enclosed
please find a proposed Iot line map prepared by Van Tuyl for Susan,'
and Ralph Tuthill. The Tuthill's presently own the parce! containing
59,927 square feet on which they constructed a house and received
a Certificate of Occupancy # Z8919. They will be conveying a strip 25
feet wide to James E. Cross on the northerly end of their lot which
strip contains 5238 square feet in exchange for a parcel to be conveyed
by James Cross on the southerly end of their property containing
27,019 square feet. The net result is that the TuthiIl's will have a
lot containing a total of 86,946 square feet as approved by the
Planning Board on April 9, 1990.
Rudy Bruer on behalf of Southold Savings Bank has requested an updated
Certificate of Occupancy reflecting the new parcel conSaining 86,946
square feet. I am enclosing herewith a copy of the Certificate of
Occupancy originally issued to the Tuthills on January 26, 19Z8, Z8919.
Also enclosed is an application for an updated Certificate of Occupancy
and my attorney's check in the sum of $50.00.
