Dear Mr. & Mrs° Burr:
At the hearing held on March 7, 1975, in the office of the Suffolk
County Department of Health Services~ Division of Public Health,
H, Lee Dennison Executive Office Building, Veterans Hemorial Highway,
Hauppauge, Ne~ York, you had an oppertunity to present your appeal
of the Department's ruling on the subject application.
In accordance with the previsions of subdivision (c), Section 7,
Article I of the Suffolk County Sanitary Code, the determination of the
Board of Review is as follows:
Based on the information submitted, the subject application be
approved provided that a public water supply is installed and all minimum
distances required by the Department standards are 'Followed.
Since the determination of the Board of Review submitted to the
Commissioner's office on March 20, 1975, has not been r~versed or
modified by her, it is therefore deemed to be the determinat}on of the
Commissioner.

Dear Mr. Terry:
We thank you for your quick response to our permit request. We did
not realize that external portions of the house fell into the fee cate-
gory. Attached find a check in the amount of $79.80 based on your com-
puted figure.
I might add here that it is the desire of both my wife Doris, and
I to comply with all local~ state, and federal regulations in helping to
maintain the natural beauty of the peninsula a&ong with the necessary
safety requirements.
We are planning to have Mr. Palmer develop finished drawings on
the interior and exterior of the house. Mr. Dick J~ will work with
us on connecting exterior telephone, electric, in conjunction
with Mr. Baker and Mr. Wall from the Gada
