1. That it is the intention of the undersigned to request a Permit from the Board of
Trustees to:
Install approximately 78 linear feet of 200-500 lb. stone rip-rap(over filter cloth)along toe of eroding
embankment(+/-48 if adjacent to James Creek and+/-30 if adjacent to Peconic Bay);and restore
embankment face with with approximately 10 cy of clean sand fill to be trucked in from an approved
upland source and planted with Cape American beach grass(18" O.C.),as depicted on the project plan
prepared by En-Consultants,dated July 25,2022.
2. That the property which is the subject of Environmental Review is located
adjacent to your property and is described as follows:
2504 Camp Mineola Road Extension,Mattituck
3. That the project which is subject to Environmental Review under Chapters
96,111 and/or 275 of the Town Code is open to public comment on:
