instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Budding
Inspector w~th the following; for new buildings or new use.
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2 Final approval of Health Dept. of water supply and sewerage d~sposal--(S-9 form or equal).
3. Approval of electncal installation from Board of F~re Underwriters
4. Commercial buddings, Industrial buildings, Multiple Residences and simpler buildings and
installations, a cerhficate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed s~te plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buddings and "pre~existing"
land uses:
1 Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings
3 Date of any hous,ng code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate

Based on the information stated hereon by the applicant and other information made available,
it is the option of this Department that th~s system with proper maintenance can be expected to
function satisfactorily and is not hkely to cause a nuisance, provided designed sewage flow is not
exceeded. Structural features are not included.

After investigation and inspection the Board finds
that applicant requests permission to build and operate
an automobile body repair shop on the north side of
MaimRoad, Laurel Dist. ~1I, ~4attituck Post Office,
New York. The findings of the Board are that this is
a favorable location for this type of operation as it is
bounded by industrial property on the north and west and
business property to the east; aaa across the street there
is a gas station. The Board agrees with the reasoning of
tha applicant.
The Board finds that the public convenience and ~lfare
aaa Justice will be served an~ the legally eshablishea or
pe=mitted use of neighborhood property and adjoining uss
districts will not be permanently or substantially injured,
and tho spirit of the Ordinanco wall be observed.
THEREFORE iT WAS RESOLVED, Harvey Bagshaw, Deep Hole
Drive, Mattituck, New York be GRANTED pe~%is~ion to build

~ew York, as applied for, subject to the followin~ conditions~
Entrance and exit driveways shall have an unrestricted
width of not less than 12 feet and not ~ore than 3O
feet and shall be located not less than 10 feet from
any property line, and shall be so laid out as to
avoid the necessity of any vehicle backing out across
any right of way.

Ail service or repair of motor vehicles, other than
