This apphcation must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features, 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. SEE KEMERSE FORM ZB4 ZONING BOARD OF APPEAr,,q ) be denied and MarJorie McDermott, Secretary After investigation and inspection the Board finds that applicant requests permission to construct addition to store with insuffiuient rear yet,I, offstraet parking, etc~ on property located on the south side of Pike Street, east of Love Lane, Mattituck. The findings of the Board are that the proposed addition to store of 4,000 sq. ft. in area, plus 2,600 sq. ft. of area' for the existing building, will necessitate a parking requirement of approximately 53 spaces which is easily accommodated in the Mattituck Park Distrtc~ public p~_rking lot, and which, under this proposal, is contiguous to the hardware store. The Board finds that the existing building is on a corner and requires no variance of setback from Pike Street. This addition will continue the present setback of the old building. App~oximately two thirds of the area of the a~dition will be used for sales and display.
