Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposals(S*9 form or equal).
3. Approval of electrical installation from Board of Fire UnderwriterS.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance fram the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2, Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent
formation required to prepare a certificate.
C. Fees: 1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00

Circa Design - Gwynne Construction - Site Plan Inspection
Henry Raynor, Jr. and Muriel Brush
Southease and southwest corners used wood chips instead of
seeding lawn area for ground cover. No liners indicated in
either of these areas.
Rear yard area showed additional plantin~ put in along the
north end of the bluestone and oil pavement. No wood chips
behind this as indic'ated on the plan.
West corner of property has small additional paved area
instead of wood chips as indicated.
Rear of model home has additional stoop not shown on site
plan.
Front of model in front of walk has no wood chips and is
sodded to the walkway. Approximately one dozen additional plantings
around model in excess of those shown on plan. Liners were used
in this area surrounding the model.
Second exit sign indicated on plan is not in existence.
Additional plantings in front of garage building at rear.
Outside of these minor details, site plan conforms. General
condition of building and upkeep are attractive and overall plan
should be considered an asset to the area. Would suggest two
year review with regard to areas woodchipped to check that weeds
do not overwhelm areas not covered with liner. Recommend that
polyetherene liners be placed on east and west sides that were
wood chipped instead of seeded and woo'd chips be replaced after
liner is down.

Dear Mr. Schwartz:
The following action was taken by the Southold Town Plarming
Board at a regular meeting held September 19, 1977.
RESOLVED that inspection has been made of the premises of
Circa Design and it conforms with the site plan. It is approved
with the provision that polyethurene liners be placed under the
wood chips in the area along the eastern driveway. There will be
a two-year review.

Dear Mr. Schwartz:
The following action was taken by the Southold Town
Planning Board at a regular meeting held December 27, 1976.
RESOLYED to approve the site plan of Gwynne Construction
Corporation (Circa Designs) dated November 1976 and revised
December 9, 1976 subject to the following conditions:
1. A polyethylene lining be placed under the wood
chips.
2. The Plarming Board will make a two-year review of
this property.
The Chairman will sign your site plan as approved on
Wednesday and a copy will be forwarded to the building
inspector with a copy of this letter.
The £ee for an application to the Planning Board for site
plan approval is ~25, which I neglected to inform you of and
receive from you.
