Instructions
A. This application must be filled in typewriter OR ~nk, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate Iocahon of ail buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. ,~pproval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Mulhple Residences and similar buildings and
installations, a certificate of Code comphance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed s~te plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses'
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

less than 2/10 of i% lead.
5. Commercial building, industrzal buzldzng, multzple residences and similar buzldings
and installatzons, a certificate of Code Complzance from architect or engineer
responszble for the buzlding.
6. Submzt Plannzng Board Approval of completed site plan requzrements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buiidings and
"pre-exmstzng" land uses:
i. Accurate survey of property showmng all property lines, streets, buildzng and
unusual natural or topographzc features.
2.. A properly completed applzcation and a consent to inspect szgned by the applzcant.
If a Certzfzcate of Occupancy zs denied, the Building Inspector shall state the
reasons therefor in writing to the applzcant.

Ladies & Gentlemen:
We are representing Bill Lindsay in connection with the refinancing
of his residence at Theresa Drive, Mattituck. The Bank involved has questioned
the Certificate of Occupancy, copy enclosed, which was issued for a "Private
one family dwelling". It is my understanding that this C.O. also included
the garage and porch overhang as shown on the attached survey. However, I
recall that it was Howard Terry's custom to not specifically itemize anything
other than the dwelling. I would therefore appreciate a letter from the
Building Department that according to your records, the C.O. includes the
garage and porch overhang. We already have C.O.s for the Deck Addition, #Z-
15837; for the inground pool, #Z-11790 and the Utility building, #Z-15836.
Please advise if there is anything additional which you require.
