Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor w~th the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage dmposa[--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

cerning an application to construct a restaurant at ~ inter-
section of County Road 27 and Sound £~enue in [~attituck.
~indly be advised that this subject property may be aftected
by the proposed extension of the Long islan3 E]:3ressway. ?his
project is, however, only in the prelimina~- planning stage
of development.
Under the new rules and r~gu=ations '.;hich ',,/ere cecently re-
vised by the Federal High%~av ..ct of t~ ~%9, the Depart~ent must
hold the prescribed num%~tr o£ t~.%%~lic i~earings. For this pro-
ject, the Depart~-nent must %,.ol~ t-:o s~[ch hearings, one called
a Corridor Public IIearing and the other called a Design Pub-
lic Hearing. It is ' ~-'-"
an~?_,.tpated that the Corridor Public
Hearing mal~ be i~!d i.~% January of 1973. IIowever, this is
only a tentative .late and is subject to change.
The Department cannot ac¢luire any propcrty in advance until
the prescribed public hearings ~re held, a recommendation
for location approYal is submitted to the Federal High~ay ~-
ministration, and approval is obtained f~om the Federal High-
way Administration. Under these procedures, ~le Department
cannot acquire the subject property even though it may be
affected by the proposed future construction, nor can we deny
the owner the use of said land until t~.e above mentioned ap-
provals are obtained.
The o%~ner of' the subject l)roperty did come to ray office to
discuss the effect of this project upon his property, llow-
ever, as mentioned above, ~%e Depar~nent cannot acquire this
property as an advance ac~.~isition.
