APPLICATION FOR BUILDING PERMIT
INSTRUCTIONS
a. This application must be completely filled in by typewriter or ~n ink and submitted in duplicate to the Building
inspector.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or pubIic streets or
areas, and giving a detailed description of layout of property must be drown on the diagram which is part of this application.
c. The work covered by this application may not be cam menced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the .applicant. Such
permit shall be kept on the premises available for inspection throughout the progress of the work.
e. No building shall be occupied or used in whole or in pa rt for any purpose whatever until a Certificate of Occupancy
shall have been gr,anted by the Building Inspector.

For the past two weeks Mr. Krukoski has been in and out
of this office, and to the Board of Appeals about the approval of
-Charlies L~ne".
I again wrote the County Department of Public works to
push this matter, so far nothing has been done about getting the
required permitt Mr. Krukoski objects to the County's requirements
and is trying to get out of doing the work they demand.
Ou~ Board will approve access as soon as the permit is
issued and the work approved.
The enclosed was sent to me to forward to you bythe
county Public Works Dept.

Bellaire, New York, to construct a building om a private
road known as "Charltes Lane" (CharleYs Lane) New Sum~folk~
New Yerk~ subject to the owner of Charle's Lane obtaining
from this Department a permit for access into C.R.26 - New
Suffolk Road.
For over three years, we have been advising the
Mr. Charles Kr~foski that he must obtain a permit under ~ection
136 of the Highway Law. However~ a month ago Mr. Kr~coski~s
son was at this Office as a representative of the owner and at
that time we were advised that the o%nner was about to construct
the street connection and would make applicat~.on under the said
section of the law. As a result of which we sent him under
date of Feb. 1, 196~ the necessary application and form of bond.
To date ~ have not heard further in the matter.
Enclosed is a copy of our letter sent to Mr. Charles Kr~oski
today. We will advise you immediately upon the issuance of a
permit to Mr. Kr~oski under Section 136 of the Highway Law so
that you may take final action on the application of James P. Howard.
