Box 11799 Southold, NY 11971 Patricia Loiacono 2387 Surf Drive Bellmore, NY 11710 April 20, 2007 Re: 2055 Bay Ave. Mattituck, NY Tax Map # 473889 144.-4-3 Dear Trustees, In May of 2000 we purchased our home at 2055 Bay Ave. in Mattituck, New York. Recent storms have damaged our dock and we have been informed that in order to make the repairs needed we must have a permit for the dock. The dock permit is still in the name of Mr. Edward Yanke, the previous owner. I respectfully request that the permit be transferred into our name. Mr. Yanke's permit states the home address as 2095 Bay Ave., however, in 2001 we were informed by the police dept. that they listed our address as 2055 Bay Ave. and recommended we inform the post office and use the 2055 address. [ also request that you amend the permit to reflect the existing dock size - a wood dock that is 22.8 ft. and a floating dock that is 23.2' x 5ft. Enclosed is a copy of the survey, the permit and a check for $100.00.
