Contractor's Name & Address: Tel. e The Purpose of this application: Redriving or replacing openwork dock or openwork pier piling, and mooring piles, removal of damaged piling, repair, resetting or replacement of floating docks, ramps, or decking, all necessitated by ice and storm damage resulting during the winter. THIS PERMIT EXPIRES ON MAY 30 OF THIS YEAR 1. No dredging or filling is permitted. e e Se Repairs to existing Structures shall be limited to replacement of timbers without change in design or dimension. No repair or replacement of bulkheads is permitted under this permit. No freshwater or tidal wetlands are to be disturbed under and circumstances. A yellow permit sign provided by this'office shall be displayed on the project at all times while under replacement. Original permit number 3 inches high must be permanently displayed on project. I, certify that the work as authorized in this special ice permit has been completed and is now ready for your inspection. Also, the permit number on this project is clearly visible. Signed - Owner Signed - Contractor -Date April 16, 1984 Called Henry Smith and told him that Mrs. Virginia Marqardt from Village Marine was into see about repairing her marina. The docks would be replaced with board only, and the piles pumped down. Henry said give an ice/storm permit and and tell her that we would need a sketch of what-she has for the files. She said she would bring in photo's also. and sketches of what they have. This was a result from the Storm in April, 1984.
