3: The purpose of this application: Redriving or"replacing openwork
dock or openwork pier piling, and mooring piles, removal of damaged
piling, repair, resetting or replacement of floating docks, ramps,
or decking, all necessitated by ice and storm damage resulting during

SPECIAL CONDITIONS
1, No dredging or filling is permitted.
2. Repairs to existing structures shall be limited to replacement of
timbers withoht change in design or dimension.
3 No repair or replacement of bulkheads is permitted under this permit'.
4. No freshwater or tidal wetlands are to be.disturbed under any
circumstances.
~5. A yellow permit sign provided by this office shall be displayed on
the project at all times while under replacement.
6. Original permit numbers (3'~ high) must be permantly displayed on
project.

Dear Mrs. Terry:
The following action was taken by the Board of Southold Town Trustees
at a regular meeting held June 1, 1982:
RESOLVED to recommend to the Southoid Town Board approval of the
wetland application ~90 of Anthony E. Vallace to construct a dock
consisting of catwalk, ramp and float at Broadwaters Cove, Cutchogue,
providing it is parallel to his right-of-way so dock does not infringe
at any time on the property to the west. All boats shall be moored
on the east side of the dock so that no boat or structure shall in-
fringe on the neighbors imaginary property line to the west at any
time~ until permission from that neighbor is obtained.

INSTRUCTIONS FOR PERMIT
A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Cterk in quadruplicate, accompanied by writter' permission from the owner of the property if not
the same as the applicant.
B. Pilot Plan must be drawn to scale on the reverse side of this application showing the location
of property, the names and locations of owners of adjoining properties, and a detailed description
of proposed project. (Note: Copies of the Pilot Plan may be made by a copy machine and attached
to the Permit Application.)
C. No .operation shall be initiated by the applicant until all permits that are required are issued.
D. A detailed statement of the rehabilitation and proposed condition of the premises after the
work is completed must be given, including a survey if required.
E. If permit Js being sought for a more sizeable excavation than the construction of a private
single dock or jetty, then this application must be accompanied by a survey and topographical map
certified by o registered land survey or Professional Engineer. The horizontal control of survey
shall be based upon an approval local coordinate system. '['he vertical control for elevation and sound-
ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological
Survey datum.
F. A statement must accompany the application describing any known prior operations conducted
on the premises in question and whether any prior licenses or permits have been issued to erect
structures or to dredge or deposit fill on said premises and whether any such permits or licenses
were ever revoked or suspended by a governmental agency.
G. A fiJing fee Of $25.00 Shall accompany the application. This filing fee includes one inspection.
