INSTRUCTIONS:
A. No operation Shall be initiated by the applicant until alt
necessary permit(s) have been issued.
This application is to be completely filled in by t}~ewriter or
clearly printed in ink and submitted to the Town Trustees Office
in quadruplicate, accompanied by written permission from the
owner of the property if not the same as the applicant. Page one
of the short assessment form must be completed and signed.

No permit shall be issued until all permit fees have been
collected by the Clerk to the Trustees.
The application must contain a pilot plan to scale including a
detailed description of the proposed project drawn on survey of
property, and an area map clearly showing the location of the
project.
If a permit is being sought for a more sizeable excavation than
the construction of a private single dock or jetty, then this
application must be accompanied by a survey and topographical mad
certified by a registered land surveyor or professional
engineer. The horizontal control of the survey shall be based
upon an approval coordinate system. The vertical control for
elevation and soundings shall be based upon U.S. Coast and
Geodetic Survey and/or U.S. Geological Survey datum. For such
projects, the Trustees may request a more detailed description of
the project site as described in Art. Ii~ Sect. 97-21 Subd.D.
Upon approval of this application, the Clerk to the Trustees
shall issue the appropriate permit(s) to the applicant. Such
permits shall be available for inspection at any time upon
request. Trustee permit numbers must be displayed on ali
structures in clearly visible characters 3 inches high.
(A~en~ment of June 25, 1987) Ail application for dredging must be
accompanied by a licensed survey showing the depth contours of
the water. This does not apply to maintenance dredging projects.
