a.This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans,accurate plot plan to scale.Fee according to schedule.
b.Plot plan showing location of lot and of buildings on premises,relationship to adjoining premises or public streets or
areas,and waterways.
c.The work covered by this application may not be commenced before issuance of Building Permit.
d.Upon approval of this application,the Building Inspector will issue a Building Permit to the applicant.Such a permit
shall be kept on the premises available for inspection throughout the work.
e.No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f.Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date.If no zoning amendments or other regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize,in writing,the extension of the permit for an
addition six months.Thereafter,a new permit shall be required.

Dear Mr. Verity,
Following up with a conversation we had earlier this year, my client
bought this property in April of this year - two lots from a 1974
subdivision.
When subdivided, the tax maps numbers were 1000-107-03-11 .02 and
11.03.
In 2002, a single deed for both lots was conveyed to the prior owners, and
a new single tax map number was assigned to the two lots together, 1000-
107-03-11 .05.
As we discussed, the two lots remain two buildable lots, although they
currently have a single tax map number. You said that before you could
issue a new CofO for the existing house, the lots should be re-deeded
separately so that two separate tax map numbers could be assigned to
them.
Attached are copies of the two new deeds, which were recorded on
8/19/2021 .
Please let me know what further steps need to be taken in order for the
Certificate of Occupancy to be issued for the residence.

Hi Connie: I am hoping you can help me because the client applied for a rental permit for the house and
he was told the house did not have a CO. Mike advised him correctly that he should have his deeds
redone to return the property to two tax map numbers. Original Tax Map # 1000-107-3-11.2 & 11.3
(House and vacant lot); new number from closing 1000-107-3-11.5; New deeds for separate lots
completed and filed at County Clerk on 8/19/2021 by Attorney Catalano...House parcel Former

Pre-CO #25737 issued on 5/26/98 pre-CO for garage & two sheds
CO 36871 on 4/24/14 for 200 amp service upgrade
CO 41820 on 2/17/21 for wood burning stove
Building Permit #45795 issued 2/8/21 kitchen addition & alteration (prior owner had not
closed out this permit and my client completed the process)
I got Trustees permit for the kitchen addition...you will probably need the Trustees C of C (let
me know if your have it- this may have been done already)
This is everything I have. What do you need?
Usually you want all the house permits in order before issuing the rental permit...looks like
everything is done and there are CO's for everything.
Please advise. If a call is easier, give me a call at your convenience.
