A. For new building or new use:
1. Final survey of property with accurate location of all buildings,property lines, streets,and unusual natural or
topographic features.
2. Final Approval from Health Dept.of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.

of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

With respect to the above mentioned premises,attached please find:
1. Two deeds, both from Adrianus Van Ryswyk and Jerina Van Ryswyk to John
Van Ryswyk and Edward J. Van Ryswyk,each dated December 7, I982-One for Lot 12, One
for Lot 13. Lot 13 reserves a life estate to Jerina Van Ryswyk.
2. Current Deed-Deed from John Van Ryswyk and Edward J. Van Ryswyk into
John Van Ryswyk and Lorraine Van Ryswyk dated September 25, 1984-one deed for the 2
parcels,Lots"12 and 13.
The deeds for Lots 12 and 13 both came from common grantor to common grantee.
Although Lot 13 includes a life estate for Jerina Van Ryswyk, she died on March 12, 1983. See
attached death certificate. Therefore, the two lots merged:before the June 1, 1983 date specified
in the provision of the Code.
Please proceed with the request for the pre-existing certificate of occupancy.
