Instructions
A. This application must be filled in typewriter OR ink, and submitted m ftw r to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Olive Hairston Murray, being duly sworn, deposes and states
as follows:
That I reside at 306 E 96th St., New York, New York and that I
am the owner of premises located on PEconic Lane, at Peconic, Town of
Southold, designated on the Suffolk County Tax Map as District 1000,
Section 74, Block 3, Lot 17.
That said premises have been used since the late 1940's as three
apartments and have been so utilized on a continuing basis to date.
At no time have any of the apartments been empty and unoccupied
for more then one year.
I submit this affidavit in support of my application for a pre-
existing Certificate of Occupancy for the subject premises for three
apartments.

Virginia L. Hairston, being duly sworn, deposes and states as
follows:
That I reside at (no #) Carroll Avenue, Peconic, New York.
That my deceased husband, Walter L. Hairston and I took title to
certain property located on the easterly side of Peconic Lane, Southold,
New York, designated on the Suffolk County Tax Map as District 1000,
Section 74, Block 3, Lot 17 in 1946. That late in the 1940's and
continuing to date the subject premises has been utilized for three
rental apartments on a continuing basis.
That in July 1984 my husband and I conveyed title to our daughter,
Olive Hairston Murray and I execute this affidavit in support of my
daughter's application for a pre-existing Certificate of Occupancy for
three apartments.

Upon request of .the Southold Town Building Department,
I made inspection of this two story one.bedroom.framed
dwelling and.found the following violations of Local.Law #1,
Housing Code, Town of Southold. I was admitted to south
entrance by Mr. Hairston and began inspection at approximately
1:25 p.m.
Kitchen: New flooring had just been installed,
refrigerator and range were not in place but are
being furnished by Mr. Hairston.
Stairway to second floor: No light switch before
ascen ing stairs - Section 2.15c. .(per..Mr. Hairston,
a pull chain will be provided to control light in
hallway on second floor providing light for stairway.)
This apartment is heated.by a standing floor model type
space heater (#1 fuel oil) which provides heat to second
story bedroom thru grilled opening in living room ceiling.
Inspection completed approximately,1:55 p.m.
