Tiffs application must be filled in by typewriter or ink and submitted to tile Building Department with the following:
For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building,.multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Michael:
Becker Morgan Group, Inc. certifies that on June 12th, 13th, 25th & 26th we conducted on-
site visits & observed construction of the Community Center and the Apartment Building
and found, to the best of our knowledge, information and belief, construction consistent
with the intent of the mechanical design documents and codes, laws and ordinances
applicable at the time of design. Our certification is limited to observations recorded on
these dates. Becker Morgan Group, Inc. has not provided regular or comprehensive on-site
construction observations.
Construction has deviated from the design documents, most notable in the conversion of
the closed-loop heat pump system to a geothermal system. However, the conversion to a
geothermal system and other observed deviations are consistent within the design intent
established by Becker Morgan Group, Inc.'s design documents.
The Health Center facilities have not been reviewed.

LLP, Consulting Engineers, dated January 14, 2000, for professional engineering services
relating to the above project, we have completed the final inspection for the subject premises.
In addition, we have reviewed inspection reports and other relevant material for conformance to
the requirements of the Town Building Department for a Certificate of Occupancy.
Based on this review, we have no objection to the issuance of a Certificate of Occupancy for the

Please be advised that as of today, STAT Fire Suppression, Inc., has hydrostatically tested the
Apartment Buildings & Parking Garage for 200lbs at two hours at the above referenced project as requested by
E & F Walsh Building Company, LLC. STAT Fire Suppression, Inc. has designed and installed the Fire Sprinkler
System In accordance with NFPA #13R and local authority codes.
At this time STAT Fire Suppression, Inc., has found the system to be in satisfactory condition and free of
leaks. STAT Fire Suppression, Inc., will not be held responsible for any damage done in these areas. Any

Peconic Landing Apartment Building--Fire Alarm System
Gentlemen:
On 8/8/02 SimplexGrinnell did a final inspection along with a final test of the Fire Alarm
System. Also all punch list items have been checked out and the System has been up and
running for the owner's use.
Therefore, as of 8/8/02 the warranty of the System has been instituted and will carry
through 8/7/03.
